Lincoln House Adult Group Home

Lincoln House is a home for five adult individuals, ages 21+ who have a history of chronic mental health issues, previous hospitalizations, and are in need of a structured environment. Referrals are made through the RI Dept. of Mental Health. The goal of Lincoln House is to assist adults in gaining greater levels of independence in the community while maintaining a safe and respectful home.

The program provides intensive clinical services and assessment, as well as case management services to participants who are living independently in the community. Case Managers support participants in negotiating and securing community resources and functioning independently.

Capacity: 5 residential care beds and 3 apartments in the community
