Lincoln House is a supportive and nurturing home designed for five men, ages 21+, who have experienced challenges with chronic mental health conditions and previous hospitalizations. Lincoln House offers a structured, compassionate environment where individuals can feel safe and empowered on their journey toward greater independence. Referrals to Lincoln House are made through the Rhode Island Department of Mental Health, ensuring that participants receive the necessary care and resources tailored to their needs.
To meet each individual’s unique needs, we offer a range of comprehensive services, including intensive clinical assessments and ongoing support from our experienced case managers. These professionals work closely with participants, helping them connect with valuable community resources and offering guidance to help them navigate life independently. Whether it's finding housing, accessing healthcare, or developing coping strategies, our case managers are there to help every step of the way.
